To join the State Children's Health Insurance Program (SCHIP), select the "Book Now" button and schedule a phone appointment with a Certified Application Assistant. Since eligibility is based on family size and family income, you will need to make a copy of a current pay stub for each working parent and a copy of your child's birth certificate. The phone call will take approximately 15 minutes.
